In my previous post I argued that the human body may react to “eating big” as it would to overfeeding, increasing energy expenditure by a certain amount. That increase seems to lead to a reduction in the caloric value of the meals during overfeeding; a reduction that seems to gravitate around 14 percent of the overfed amount.
And what is the overfed amount? Let us assume that your daily calorie intake to maintain your current body weight is 2,000 calories. However, one day you consume 1,000 calories, and the next 3,000 – adding up to 4,000 calories in 2 days. This amounts to 2,000 calories per day on average, the weight maintenance amount; but the extra 1,000 on the second day is perceived by your body as overfeeding. So 140 calories are “lost”.
The mechanisms by which this could happen are not entirely clear. Some studies contain clues; one example is the 2002 study conducted with mice by Anson and colleagues (), from which the graphs below were taken.
In the graphs above AL refers to ad libitum feeding, LDF to limited daily feeding (40 percent less than AL), IF to intermittent (alternate-day) fasting, and PF to pair-fed mice that were provided daily with a food allotment equal to the average daily intake of mice in the IF group. PF was added a control condition; in practice, the 2-day food consumption was about the same in AL, IF and PF.
After a 20-week period, intermittent fasting was associated with the lowest blood glucose and insulin concentrations (graphs a and b), and the highest concentrations of insulin growth factor 1 and ketones (graphs c and d). These seem to be fairly positive outcomes. In humans, they would normally be associated with metabolic improvements and body fat loss.
Let us go back to the 14 percent advantage of eating little and then a lot; a pattern of eating that can be implemented though intermittent fasting, as well as other approaches.
So, as we have seen in the previous post (), it seems that if you consume the same number of calories, but you do that while alternating between underfeeding and overfeeding, you actually “absorb” 14 percent fewer calories – with that percentage applied to the extra calorie intake above the amount needed for weight maintenance.
And here is a critical point, which I already hinted at in the previous post (): energy expenditure is not significantly reduced by underfeeding, as long as it is short-term underfeeding – e.g., about 24 h or less. So you don’t “gain back” the calories due to a possible reduction in energy expenditure in the (relatively short) underfeeding period.
What do 140 calories mean in terms of fat loss? Just divide that amount by 9 to get an estimate; about 15 g of fat lost. This is about 1 lb per month, and 12 lbs per year. Does one lose muscle due to this, in addition to body fat? A period of underfeeding of about 24 h or less should not be enough to lead to loss of muscle, as long as one doesn’t do glycogen-depleting exercise during that period ().
Sounds good? It actually gets better. Underfeeding tends to increase the body’s receptivity to both micronutrients and macronutrients. This applies to protein, carbohydrates, vitamins etc. For example, the activity of liver and muscle glycogen synthase is significantly increased by underfeeding (the scientific term is “phosphorylation”), particularly carbohydrate underfeeding, effectively raising the insulin sensitivity of those tissues.
The same happens, in general terms, with a host of other tissues and nutrients; often mediated by enzymes. This means that after a short period of underfeeding your body is primed to absorb micronutrients and macronutrients more effectively, even as it uses up some extra calories – leading to a 14 percent increase in energy expenditure.
There are many ways in which this can be achieved. Intermittent fasting is one of them; with 16-h to 24-h fasts, for example. Intermittent calorie restriction is another; e.g., with a 1/3 and 2/3 calorie consumption pattern across two-day periods. Yet another is intermittent carbohydrate restriction, with other macronutrients kept more or less constant.
If the same amount of food is consumed, there is evidence suggesting that such practices would lead to body weight preservation with improved body composition – same body weight, but reduced fat mass. This is what the study by Anson and colleagues, mentioned earlier, suggested ().
A 2005 study by Heilbronn and colleagues on alternate day fasting by humans suggested a small decrease in body weight (); although the loss was clearly mostly of fat mass. Interestingly, this study with nonobese humans suggested a massive decrease in fasting insulin, much like the mice study by Anson and colleagues.
Having said all of the above, there are several people who gain body fat by alternating between eating little and a lot. Why is that? The most likely reason is that when they eat a lot their caloric intake exceeds the increased energy expenditure.

The 14-percent advantage of eating little and then a lot: Is it real?
When you look at the literature on overfeeding, you see a number over and over again – 14 percent. That is approximately the increase in energy expenditure you get when you overfeed people; that is, when you feed people more calories that they need to maintain their current weight.
This phenomenon is related to another interesting one: the nonlinear increase in body weight and fat mass following overfeeding after a period starvation, illustrated by the top graph below from an article by Kevin Hall (). The data for the squares on the top graph is from the Minnesota Starvation Experiment (). The graph at the bottom is based mostly on the results of a simulation, and doesn’t clearly reflect the phenomenon.
Due to the significant amount of weight lost in what is called above the semistarvation stage (SS), the controlled refeeding period (CR) actually involved significant overfeeding. Nevertheless, weight was not gained right away, due to a sharp increase in energy expenditure. That is illustrated by the U-curve shape of the weight gain in response to overfeeding. Initially the gain is minimal, increasing over time, and continuing through the ad libitum refeeding stage (ALR).
Interestingly, overfeeding leads to increased energy expenditure almost immediately after it starts happening. It seems that even one single unusually big meal will significantly increase energy expenditure. Also, the 14 percent is usually associated with meals with a balanced amount of macronutrients. That percentage seems to go down if the balance is significantly shifted toward dietary fat (), probably because the metabolic “cost” of converting dietary fat into body fat is low. In other words, large meals with a lot of fat in them tend to cause a reduced increase in energy expenditure – less than 14 percent. Shifting the balance to protein appears to have the opposite effect, increasing energy expenditure even more, probably because protein is the jack-of-all-trades among macronutrients ().
The calorie surplus used in experiments where the 14 percent increase in energy expenditure is observed is normally around 1,000 calories, but the percentage seems to hold steady when people are overfed to different degrees () (). Let us assume that one is overfed 1,000 calories. What happens? About 140 calories are “lost” due to overfeeding.

It allows for some reasonable speculation, based on a simple pattern: when you alternate between underfeeding and overfeeding, you reduce food consumption for short period of time (usually less than 24 h), and then eat big, because you are hungry.

So you have the potential here for some steady fat loss without a reduction in caloric intake. Keeping a calorie intake up above a certain point is more important than many people think, because a calorie intake that is too low may lead to nutrient deficiencies (). This is possibly one of the reasons why carrying a bit of extra weight is associated with increased longevity in relatively sedentary populations ().
Is this 14-percent effect real, or just another mirage? If yes, what does it possibly translate into in terms of fat loss? More on these issues is coming in the next post.

If your NEAT is low, maybe you should chill
I wrote most of this post a while ago, and then forgot about it. The recent blogosphere storm of comments regarding cold-induced thermogenesis caught me by surprise (), and provided a motivation to get this post out. Contrary to popular perception, I guess, cold-induced thermogenesis is an extensively researched topic. Some reasonably well cited references are linked here.
Let us backtrack a bit. When people say that they want to lose weight, usually what they really want is to lose is body fat. However, they frequently do things that make them lose what they do not want – muscle glycogen, water, and even some muscle protein. Physical activity in general depletes muscle glycogen; even aerobic physical activity.
Walking, for example, depletes muscle glycogen; but slowly, and proportionally to how fast one walks. Weight training and sprints deplete muscle glycogen much faster. Whatever depletes muscle glycogen also lowers the amount of water stored in myocytes (muscle cells), effectively reducing muscle mass. Depleted muscle glycogen needs to be replenished; protein and carbohydrates are the sources. If you deplete muscle glycogen through strength training, this will provide a strong stimulus for glycogen replenishment and thus muscle growth, even beyond the original level – a phenomenon called supercompensation ().
In conjunction with strength training, situations in which one burns mostly fat, and very little glycogen, should be at the top of the list for those wishing to lose weight by losing body fat and nothing else. These are not very common though. One example is nonexercise activity thermogenesis (NEAT), or heat generation from nonexercise activities such as fidgeting (). There is a great deal of variation in NEAT across individuals; for some it is high, for others it is annoyingly low.
Walking slowly is almost as good as NEAT for body fat burning, when done in conjunction with strength training. Up the pace a bit though, and you’ll be burning more muscle glycogen. But if you walk slowly you don’t burn that much body fat per unit of time. If you walk a bit faster you’ll burn more fat, but also more glycogen. C’mon, there is no way to win in this game!
This is why being physically active, in a “non-exercise way”, seems to be so important for health; together with strength training, limiting calorie intake, and all the while having a nutritious diet. These are not very common things in modern urban environments. Long term, there isn’t a lot of margin for error. It is ultimately a game of small numbers in the short term, played over long periods of time.
But there is an alternative if your NEAT is low – just chill. That is, another situation in which one can burn mostly fat, and very little glycogen, is exposure to mildly cold temperatures, but above the level that induces shivering (mild cold: 16 degrees Celsius or so; about 60 degrees Fahrenheit). Shivering in general, and particularly intense shivering, is associated with levels of muscle activity that would induce glycogen depletion () (). If muscle glycogen depletion happens while one is fasting, liver glycogen will be used to replenish muscle glycogen, and also to supply the needs of the brain – which is always hungry for glucose.
As the liver glycogen tank goes down beyond a certain point, and no protein or carbohydrates are eaten, the body will use amino acids from muscle to produce glucose. Muscle glycogen will be locked until it is needed. Interesting eh!? The body sacrifices muscle protein but doesn’t tap into muscle glycogen, which is only used to fuel violent muscle contractions. We are talking about fight-or-flight responses here. From an evolutionary perspective, sacrificing some muscle beats losing a lot of it to a predator any day.
Cold-induced thermogenesis is a very interesting phenomenon. The figure below, where open circles represent lean and closed circles obese folks, shows that it leads to different responses in lean and obese folks, and also that it presents a lot of variation across different individuals (like NEAT). This type of thermogenesis actually seems to be strongly associated with an increase in NEAT (); although it seems to also be associated with futile cycles used by the body to generate heat without any movement, as in thermogenesis during hibernation in certain animals () (). Having more brown fat as an adult, or being able to make brown fat more easily, is associated with more cold-induced thermogenesis; and also with a lower obesity risk.
In fact, cold-induced thermogenesis leads to an increase in energy expenditure that is comparable with that of another major energy sinkhole – overfeeding () (). Unlike overfeeding though, cold-induced thermogenesis does not require calories to go in. And, no, you don’t burn more than you take in with overfeeding.
How can one burn fat via cold-induced thermogenesis? Here are some ideas. Set the home thermostat to a mildly cold temperature in the winter (this will also save you some money). When it is a little cooler than normal, don’t wear heavy clothes. Take mildly cold showers, or end a warm shower with some mildly cold water.
What about more extreme cold exposure? It should be no surprise that one would feel pretty good after a dip in ice-cold water; that is, if the person does not suffer from a glycogen storage disease (e.g., McArdle's disease). At least in theory, that type of cold exposure should induce whole-body muscle glycogen depletion, just like an intense whole-body exercise session, with the resulting hormonal changes ().
Growth hormone should be up after that, perhaps for hours. Done right after weight training, or intense exercise, it may have a boosting effect on the hormonal response. But if you do that in the recovery phase (e.g., several hours after the weight training session), it should impair muscle recovery. It would be a bit like doing another strength training session, when the body is trying to recover from the previous one.

Is working standing up too expensive? It could cost you as little as $10
Spending too much time sitting down is clearly unnatural, particularly if you sit down on very comfortable chairs. Sitting down per se is probably natural, given the human anatomy, but not sitting down for hours in the same position. Also, comfortable furniture is an apparently benign Neolithic invention, but over several years it may stealthily contributed to the metabolic syndrome and the diseases of civilization.
Getting an elevated workstation may be a bit expensive. At work, you may have to go through a bit of a battle with your employer to get it (unless you are "teh boz"), only to find out that having to work standing up all the time is not what you really wanted. That may not be very natural either. So what is one to do? One possible solution is to buy a small foldable plastic table (or chair) like the one on the figure below, which may cost you less than $10, and put it on your work desk. I have been doing this for quite a while now, and it works fine for me.
The photo above shows a laptop computer. Nevertheless, you can use this table-over-table approach with a desktop computer as well. And you still keep the space under the foldable table, which you can use to place other items. With a desktop computer this approach would probably require two foldable tables to elevate the screen, keyboard, and mouse. This approach also works for reading documents and writing with a pen or pencil; just put a thick sheet of paper on the foldable table to make a flat surface (if the foldable table’s surface is not flat already). And you don’t have to be standing up all the time; you can sit down as well after removing the foldable table. It takes me about 5 seconds to do or undo this setup.
When you sit down, you may want to consider using a pillow like the one on the photo to force yourself to sit upright. (You can use it as shown, or place the pillow flat on the chair and sit on its edge.) Sitting on a very comfy chair with back support prevents you from using the various abdominal and back muscles needed to maintain posture. As a result, you may find yourself unusually prone to low back injuries and suffering from “mysterious” abdominal discomfort. You will also very likely decrease your nonexercise activity thermogenesis (NEAT), which is a major calorie expenditure regulator.
With posture stabilization muscles, as with almost everything else in the human body, the reality is this: if you don’t use them, you lose them.

Low nonexercise activity thermogenesis: Uncooperative genes or comfy furniture?
The degree of nonexercise activity thermogenesis (NEAT) seems to a major factor influencing the amount of fat gained or lost by an individual. It also seems to be strongly influenced by genetics, because NEAT is largely due to involuntary activities like fidgeting.
But why should this be?
The degree to which different individuals will develop diseases of civilization in response to consumption of refined carbohydrate-rich foods can also be seen as influenced by genetics. After all, there are many people who eat those foods and are thin and healthy, and that appears to be in part a family trait. But whether we consume those products or not is largely within our control.
So, it is quite possible that NEAT is influenced by genetics, but the fact that NEAT is low in so many people should be a red flag. In the same way that the fact that so many people who eat refined carbohydrate-rich foods are obese should be a red flag. Moreover, modern isolated hunter-gatherers tend to have low levels of body fat. Given the importance of NEAT for body fat regulation, it is not unreasonable to assume that NEAT is elevated in hunter-gatherers, compared to modern urbanites. Hunter-gatherers live more like our Paleolithic ancestors than modern urbanites.
True genetic diseases, caused by recent harmful mutations, are usually rare. If low NEAT were truly a genetic “disease”, those with low NEAT should be a small minority. That is not the case. It is more likely that the low NEAT that we see in modern urbanites is due to a maladaptation of our Stone Age body to modern life, in the same way that our Stone Age body is maladapted to the consumption of foods rich in refined grains and seeds.
What could have increased NEAT among our Paleolithic ancestors, and among modern isolated hunter-gatherers?
One thing that comes to mind is lack of comfortable furniture, particularly comfortable chairs (photo below from: prlog.org). It is quite possible that our Paleolithic ancestors invented some rudimentary forms of furniture, but they would have been much less comfortable than modern furniture used in most offices and homes. The padding of comfy office chairs is not very easy to replicate with stones, leaves, wood, or even animal hides. You need engineering to design it; you need industry to produce that kind of thing.
I have been doing a little experiment with myself, where I do things that force me to sit tall and stand while working in my office, instead of sitting back and “relaxing”. Things like putting a pillow on the chair so that I cannot rest my back on it, or placing my computer on an elevated surface so that I am forced to work while standing up. I tend to move a lot more when I do those things, and the movement is largely involuntary. These are small but constant movements, a bit like fidgeting. (It would be interesting to tape myself and actually quantify the amount of movement.)

Both obesity and low-back problems are widespread among modern urbanites. Yet, from an evolutionary perspective, they should not be. They likely impaired survival success among our ancestors, and thus impaired their reproductive success. Evolution “gets angry” at these things; over time it wipes them out. In my reading of studies of hunter-gatherers, I don’t recall a single instance in which obesity and low-back problems were described as being widespread.
But why should this be?
The degree to which different individuals will develop diseases of civilization in response to consumption of refined carbohydrate-rich foods can also be seen as influenced by genetics. After all, there are many people who eat those foods and are thin and healthy, and that appears to be in part a family trait. But whether we consume those products or not is largely within our control.
So, it is quite possible that NEAT is influenced by genetics, but the fact that NEAT is low in so many people should be a red flag. In the same way that the fact that so many people who eat refined carbohydrate-rich foods are obese should be a red flag. Moreover, modern isolated hunter-gatherers tend to have low levels of body fat. Given the importance of NEAT for body fat regulation, it is not unreasonable to assume that NEAT is elevated in hunter-gatherers, compared to modern urbanites. Hunter-gatherers live more like our Paleolithic ancestors than modern urbanites.
True genetic diseases, caused by recent harmful mutations, are usually rare. If low NEAT were truly a genetic “disease”, those with low NEAT should be a small minority. That is not the case. It is more likely that the low NEAT that we see in modern urbanites is due to a maladaptation of our Stone Age body to modern life, in the same way that our Stone Age body is maladapted to the consumption of foods rich in refined grains and seeds.
What could have increased NEAT among our Paleolithic ancestors, and among modern isolated hunter-gatherers?
One thing that comes to mind is lack of comfortable furniture, particularly comfortable chairs (photo below from: prlog.org). It is quite possible that our Paleolithic ancestors invented some rudimentary forms of furniture, but they would have been much less comfortable than modern furniture used in most offices and homes. The padding of comfy office chairs is not very easy to replicate with stones, leaves, wood, or even animal hides. You need engineering to design it; you need industry to produce that kind of thing.
I have been doing a little experiment with myself, where I do things that force me to sit tall and stand while working in my office, instead of sitting back and “relaxing”. Things like putting a pillow on the chair so that I cannot rest my back on it, or placing my computer on an elevated surface so that I am forced to work while standing up. I tend to move a lot more when I do those things, and the movement is largely involuntary. These are small but constant movements, a bit like fidgeting. (It would be interesting to tape myself and actually quantify the amount of movement.)

Nonexercise activities like fidgeting may account for a 1,000 percent difference in body fat gain! NEAT eh?
Some studies become classics in their fields and yet are largely missed by the popular media. This seems to be what happened with a study by Levine and colleagues (1999; full reference and link at the end of this post), which looked at the role that nonexercise activity thermogenesis (NEAT) plays in fat gain suppression. You have probably seen on the web claims that overeating leads to fat loss, because overeating increases one’s basal metabolic rate. There are also claims that food has a powerful thermic effect, due to the energy needed for digestion, absorption and storage of nutrients; this is also claimed to lead to fat loss. There is some truth to these claims, but the related effects are very small compared with the effects of NEAT.
Ever wonder why there are some folks who seem to eat whatever they want, and never get fat? As it turns out, it may be primarily due to NEAT!
NEAT is associated with fidgeting, maintenance of posture, shifting position, pacing, and other involuntary light physical activities. The main finding of this study was that NEAT accounted for a massive amount of the difference in body fat gain among the participants in the study. The participants were 12 males and 4 females, ranging in age from 25 to 36 years. These healthy and lean participants were fed 1,000 kilocalories per day in excess of their weight-maintenance requirements, for a period of 8 weeks. See figure below; click on it to enlarge.
Fat gain varied more than 10-fold among the participants (or more than 1,000 percent), ranging from a gain of only 0.36 kg (0.79 lbs) to a gain of 4.23 kg (9.33 lbs). As you can see, NEAT explains a lot of the variance in the fat gain variable, which is indicated by the highly statistically significant negative correlation (-0.77). Its effect dwarfs those related to basal metabolic rate and food-induced thermogenesis, neither of which was statistically significant.
How can one use this finding in practice? This research indirectly suggests that moving often throughout the day may have a significant additive long term effect on fat gain suppression. It is reasonable to expect a similar effect on fat loss. And this effect may be stealthy enough to prevent the body from reacting to fat loss by significantly lowering its basal metabolic rate. (Yes, while the increase in basal metabolic rate is trivial in response to overfeeding, the decrease in this rate is nontrivial in response to underfeeding. Essentially the body is much more “concerned” about starving than fattening up.)
The bad news is that it is not easy to mimic the effects of NEAT through voluntary activities. The authors of the study estimated that the maximum increase in NEAT detected in the study (692 kcal/day) would be equivalent to a 15-minute walk every waking hour of every single day! (This other study focuses specifically on fidgeting.) Clearly NEAT has a powerful effect on weight loss, which is not easy to match with voluntary pacing, standing up etc. Moreover, females seem to benefit less from NEAT, because they seem to engage in fewer NEAT-related activities than men. The four lowest NEAT values in the study corresponded to the four female participants.
Nevertheless, if you have a desk job, like I do, you may want to stand up and pace for a few seconds every 30 minutes. You may also want to stand up while you talk on the phone. You may want to shift position from time to time; e.g., sitting at the edge of the chair for a few minutes every hour, without back support. And so on. These actions may take you a bit closer to the lifestyle of our Paleolithic ancestors, who were not sitting down motionless the whole day. Try also eating more like they did and, over a year, the results may be dramatic!
